  3. Year of birth - enter a number between 1600 and 2000 for the year of birth or leave blank to not consider year of birth - enter +/- from 0 to 10 to give year of birth span (if 0 or null it is ignored).
  4. Month of birth - enter a number between 1 and 12 for the year of birth or leave blank to not consider month of birth.
